Care Management Services

  • Review of medical, legal, physical, social, and financial concerns
  • Housing, residential or assisted living referrals
  • Transportation resources
  • In-home safety and modification review
  • Review of short, intermediate, and long term needs and goals
  • Care planning, caregiver and family education
  • Linkage to community and support resources
  • Follow up and on-going reevaluation
